Student Athletic Forms
Every child participating in a diocesan-sponsored varsity or junior varsity sport at Our Lady's Christian School must have a Permission Form and a Sports Responsibility Form on file before engaging in any team activity. All athletes are also asked to complete the questionnaires listed below.

Intramural sports and fourth-grade basketball fees are handled separately as those seasons unfold. There are no required forms unless your child is participating in a diocesan-sponsored varsity or junior varsity sport.
